Benefits Eligibility and Enrollment Forms
Generally, positions lasting more than six months that are at least 50% full-time are eligible for benefits. Coverage begins the first day of the month following the date of hire. Spouse or registered partner and dependent children are eligible for coverage.The majority of insurance premiums are covered by CWU. Employee premiums vary with plan choice and dependent coverage.
If you choose to waive medical coverage, indicate so on the enrollment form. Dental coverage and retirement plan participation may not be waived. Direct questions to (509) 963-1202 or hr@cwu.edu.

Medical/Dental Insurance
Completed enrollment forms must be returned to HR within 31 days of hire.

Insuring Family Members: If covering family members, dependent verification documentation are required and must be submitted with enrollment form.
Surcharges: A $25 surcharge is added to monthly premiums if you or a family member uses tobacco and a $50 surcharge is added to monthly premiums if a spouse/partner is enrolled on coverage and has chosen to not enroll in comparable medical coverage through his/her own employer.

Life Insurance
2. Your Enrollment (PDF) must be completed within 31 days of hire [Required]
A $35,000 term life insurance policy is provided to benefit eligible employees. Additional coverage is available.

Long Term Disability (LTD) Insurance
Completed enrollment forms must be returned to HR within 31 days of hire.
CWU provides basic coverage after 90 days of disability for all benefits eligible employees and additional coverage replacing 60% of salary for faculty with tenure-track or one-year appointments. Part-time faculty may purchase the additional coverage.

Retirement
An election form must be submitted and enrollment completed within 30 days of hire/date of eligibility.
A decision must be made between the Central Washington University Retirement Plan (CWURP) and the Washington State Department of Retirement Systems Teachers Retirement Plan (TRS 3).

Central Washington University Retirement Plan (CWURP)The CWURP is a defined contribution plan with required contribution rates based on age, immediate vesting, 100% matching contribution by the university, and a variety of investment options.

Teachers' Retirement System Plan 3 (TRS 3)
The TRS 3 plan consists of a defined contribution component contributed to by the employee and a defined benefit component contributed to by the university.
